Q: “It’s been weeks since my ceremony, and I’m experiencing depression, anxiety and disconnection that feels worse than before I started this work. Is this normal integration, or could the medicine have triggered something harmful?”

A: I hear you. I’ve witnessed this territory before. The weeks following ceremony can sometimes feel like being suspended between worlds.

The truth is that while most people don’t experience lasting difficulties after working with plant medicines, but serious problems and disruptions do sometimes arise. For too long, our community has avoided this topic, but wisdom requires us to also acknowledge the shadows.

Recent research involving 608 people who experienced prolonged challenges (Psychedelics-Associated Psychological Distress) reveals patterns that mirror what you’re feeling. Among those who reported lasting difficulties, most faced emotional upheaval like severe anxiety and depression. Many struggled with existential confusion about reality itself. Others found themselves questioning fundamental personal beliefs. For one third of these individuals, the difficulties persisted beyond a year. This isn’t gentle unfolding; this is genuine suffering that demands our compassion and understanding.

Another study examined 32 people who had severe responses (Psychological Injury After Psychedelics). Nearly 9 out of 10 experienced new or worsened anxiety. More than a third received new psychiatric diagnoses. When researchers spoke with those most affected they found these were genuine psychological injuries that disrupted relationships, work, and daily functioning.

Both studies revealed patterns about who might be more susceptible to these difficult outcomes. Understanding these patterns helps in recognizing when extra care might be needed.

Environmental factors:ย Unsafe settings create risk. Unknown doses leave people unable to prepare appropriately. Chaotic circumstances during the experience can create trauma rather than healing. Complete isolation without proper support can amplify difficulties.

Personal vulnerabilities: People under 25 appear particularly susceptible, likely because their brains are still developing and may be less equipped to handle overwhelming experiences. Childhood trauma showed up in 40% of cases involving extended difficulties, suggesting that unresolved wounds can be torn open rather than healed when the timing or support isn’t right. Pre-existing mental health conditions and recent major life upheavals can affect capacity to safely navigate and interpret experiences.

Seek immediate help if you’re having thoughts of self-harm, experiencing psychotic symptoms, feeling intense paranoia, anxiety or depression persist beyond what feels manageable, you feel disconnected from yourself in ways that feel foreign and disturbing, or finding yourself unable to care for yourself or function in basic ways. These symptoms require professional intervention, not just integration support. The nervous system knows the difference between challenging growth and genuine harm. Trust what yours is telling you.

There’s a crucial difference between difficult experiences that serve our growth and those that cause harm. Challenging experiences that serve healing typically carry some sense of meaning, even when painful; a thread of understanding, however thin, that connects expansion or understanding. Harmful experiences tend to feel disconnected from a sense of purpose or learning and create more confusion than clarity.

Severe negative responses can happen when powerful medicines open us up at times when we can’t safely navigate that openness. Usually due to environmental chaos, poor timing, inappropriate dosing, or simply not being ready for what was revealed or experienced. Sometimes psychedelics show us territories which may be different from what we thought we needed or were ready to face.

If difficulties persist beyond several days, they often require support beyond traditional psychedelic integration work. You might need trauma-informed therapy to help process what emerged, psychiatric evaluation if symptoms feel overwhelming or dangerous, or body-based or energetic healing modalities to help regulate a dysregulated nervous system, or sometimes medication to stabilize us so healing can take place.

The researchers found that when people received appropriate interventions, most return to wellbeing, though some chose to step away from future psychedelic work.

Psychedelics can cause genuine psychological distress, particularly when multiple risk factors align. Acknowledging this honors the full spectrum of what working with consciousness can bring. True reverence for these teachers includes accepting their capacity for both healing and harm.
